How/why would a graphic imported by reference not show up in the LOR?
I've found at least 4 graphics in two of my books that aren't listed in
the generated LOR (All are imported by reference.)

I don't use tables very often, but I have a small one that has a few
narrow columns that contain asterisks to indicate whether a condition is

Esc - h (Escape key, hyphen, h) is what I've always used. Works for me!
