I had not seen your query previous to this posting.
This spacing is controlled by the TableFootnote box that exists by default
on the reference page (usually the first one). If it has been deleted you
can just create a new blank portrait document (File>New>Document>Portrait).
Go the first refer
If you have several chapters in the
same book, copy the header or footer from another chapter. If that
doesn't help, then copy the reference graphic from another chapter
to this chapter.
