Rebecca, Art's approach works. I just tested this by inserting a PDF fillable form in a PDF created from FrameMaker. I was under the impression you could insert a fillable form into a Framemaker book after it had been converted to PDF. I keep trying and the fields go away. If anyone can help me, I would be grateful!!! I believe you can add a fillable PDF form to the .PDF of a FrameMaker book after the fact, with Acrobat, but I don't think you can embed a working form into a FM file. FM treats imported .pdfs as graphics. A third party outfit, such as Microtype, may make some kind of add-in, but if you were going to keep it simple, I think you essentially want to merge .pdfs late in the production steps, after both the .pdf book and .pdf form have been created.
