Yes, I'll second Shlomo's answer: IndexRef from Sundorne,
a $US 25 ($CD
Try IndexRef (http://sundorne.com/FrameMaker/IndexRef/indexref.htm).
It's a little utility I wrote to solve this problem. There is a 30 day
evaluation available so you make sure it works before buying it.
