Look at the groff_man(7) manual page for a description of the format of
manpages. Or look in the source, e.g.

/usr/src/share/man/man4/ichsmb.4

(Which, btw, looks a lot like what you created below)

The usual way to supply patches is through the send-pr(1) mechanism.
