Oops! Yes, the ol' /dev/MAKEDEV script. Panic has a way of wiping
one's memory. :)

/dev/MAKEDEV ad4

I believe will recreate the device for you, although ICBW as I haven't
had much to do with any 4.x systems for quite some time, and what with
the dynamic /dev/ stuff nowadays, those neurons are beeing fairly rapidly
recycled.
