There seems to be a problem with ICQ and privacy, it doesn't refresh to show 
blocked names (red circle with a line though it) when going from normal 
available status to invisible status until you go to the privacy settings and 
pull up the accuount, then it does (without the need to change the privacy 
setting itself).  

I don't know if it is just a lack of refreshing the screen/list or actually a 
failure in changing the privacy setting till you bring up the privacy window 
and look at the settings for the account in question.  As soon as you bring up 
the privacy window the buddies get red blocked icons on them.

We have found the bug's location in gaim-1.5.0 source code, file
src/protocols/oscar/oscar.c, line 5821. 

In function oscar_set_away_icq(), somebody add a decision for the
Invisible mode in ICQ, that force the privacy to 4 or 3 mode when user
change the online_mode for ICQ account.
I think this one want to use the privacy Block all user to implement the
online_mode Invisible, because there is not a valid Invisible
online_mode to set in Protocol Oscar gaim source version, but his decision
code is unconsidered.

It seems to be a more general problem. When going to invisible mode gaim
randomly marks contacts with red circle (though, me too don't know if it is
only visual problem). I always have to go to privacy settings and reselect
the Allow only contacts below - then the proper contacts are marked
blocked.
The same problem appears when switching back to available mode. Gaim again
seems to randomly unmark some contacts, while some are left with red
circle. To make it even worse, these contacts are really blocked and in
privacy settings the option Allow only the users below is selected.
Next part of bug was discovered with gaim - miranda, so it may be
miranda's bug as well.
When you try to send such a blocked contact a message, it will be
received, but the other site will still see you as offline. I don't know
who is responsible for this, but a standard behavior (of official icq
client) is, that when a message from invisible contact is received, the
contact is marked invisible.
