the false assumption you have to have a lot of space to move the mouse
around. In actuality your arm/wrist should remain stationary while you
use your wrist to move the mouse left, right

Yes, most three button mice are like that. If you turn the scroll
wheel it acts as a z axis. If you Press the wheel down it sends a
mouse button clicked event to the application. Most of the time
clicking the wheel is the middle button or button 3.
