The only user of IFCVT_EXTRA_FIELDS is FR-V -- and it's not even using
the macro to define extra fields...
This patch removes IFCVT_EXTRA_FIELDS and replaces the related
IFCVT_INIT_EXTRA_FIELDS with IFCVT_MACHDEP_INIT.
Bootstrappedtested on x86_64-unknown-linux-gnu, and buildtested with
