FEATURES=maketest runs 'make test' if the Makefiles would automatically do so, whereas FEATURES=test supports any custom code in src_test. Since early 2006, they are no longer different. To see the full history of why they both existed, you'll need to find the old Portage SVN repo, as the Git migration was done without the history prior to August 28 2005.
