Sounds like I just need to emerge egroupware.
Per your advise, I tried:
emerge --info | grep -i pdo
I did get my USE flag as output, and it looked sane.

After a recent emerge --sync, I did a new emerge of php with --newuse...
after adding pdo to my tags list.
