We do it the same as Toni and from the CAP Uniform Labeling of slides and
Blocks presentation I attended at NSH that is what CAP came up with for uniform
labeling (S14-1234 A1, A2, A3, B1, B2, etc...).

I responded to jw privately but want to show my support for Hazel or anyone
else that is facing a CAP inspection. I think Hazel and everyone that posts
that CAP is in their institution is looking for moral support and I sent Hazel
a message right away saying Good luck. I hope Hazel or anyone
The below statement is correct and our last Joint Commission inpector clarified
that the 2 patient identifier rule only applies to the specimen containers
coming in the door. Once they are accessioned in Path you can label slides and
blocks with only the accession number and do not need the 2
I agree with you Pam. We changed from Accu Edge to the Thermo Premiers several
years ago. The Thermo Premiers gives us the opportunity to cut more blocks
than the Accu Edge. We love them!!!

For HE's, a log goes to the Pathologist with the most slides for the day and
he checks if the technical quality of the slides is good or not and initials.

We do ours manually also (about 20 different antibodies). We use a combo of
BioGenex and Dako. I like both set ups, but Dako is more expensive to perform
than BioGenex. I like that you can buy the in dividual components from
BioGenex instead of a whole ket from DAKO.

A For filing reasons. When we file in the boxes on top of microtome they are
right side up and for final filing in the plastic drawers writing is right side

We have the Meditech Computer system and we have built a data section between
Micro and Diagnosis namde Synoptic Tumor Protocol nd have canned texts
available for all tumor types that pull in the required information needed.
All the Pathologist's have to do is enter the information and hit

I know of a lab that was dinged by CAP for doing this. The inspector said that
was not a suitable solution to the problem. The lab had to change their

We also use the Aloetouch gloves for all aspects of Histology and have had no
issues with them. They fit like a glove and are very durable but continue to
have good flexability.
Luckily Medline is on contract with HPG (our buying group)!!!

I disagree. I take my molds right out of the holding chamber and use warm and
have hardly any leakage and don't have to hardly scrape.
However, the other techs do have leakage and have to scrape alot off.
I think it is just in the embedders technique.
