Re: [Histonet] EPIC Beaker
Good afternoon! We just went live with Epic Beaker June 2013 and it's been one fat mess. I have plenty of information for anyone interested. It seems there were lots of things that weren't considered for AP with Epic beaker. We have faced a lot of problems with go-live. My first piece of advice is to keep all you pathologists VERY involved with the build structure and to consult with Optime frequently. We've had the most difficulty with Optime and Beaker talking and specimens getting lost in Epic. Or, we've had AP specimens get lost in core lab, and everything has to be reordered to get things moving. Please feel free to contact me with any questions. The build is a big beast, and we didn't have much success with our go-live because there were many things we didn't really know to consider. Re: [Histonet] Tissue Processor Validation
Yes, we do a few validation runs. Nothing too extreme, however we gross some extra patient tissue and do a small run, then increase the amount of blocks processed until we are ready to run cases for patient diagnosis. It really only takes a week or two. After those blocks are cut, then we compare them to the previous processor results and make changes if neccessary. Re: [Histonet] Unstained slides
Martha, I have had experience in three different labs. The unstained slides can pile up quickly, especially when you are using charged slides that are just in case slides for sendouts, IHC, etc. For core biopsies (breasts, fine needles, prostate, etc.) we cut levels and put up at least five of the in between sections to keep on hand in case. We maybe use these slides 10 percent of the time and toss the rest after 3-4 weeks. It can seem wasteful, however if you have a small core, you must save precious tissue. Re: [Histonet] Uncertified Histotechs
Well, my opinion is based on my own experience. I was hired as a histotech with no prior histology experience besides what I completed in the laboratory in college (This was only four years ago in Columbia, MO). I trained on the job, then moved to be a histotech in Cedar Rapids Iowa. I then decided that this was what I wanted to do. So, I bought the textbooks, read them, and sat for my HTL exam (the route 2 method). I graduated top of my class with a triple major in biology, life science and chemistry. Once I became an HTL, I decided to move along my career. Age 27, only in histology for three years, I am the anatomic pathology supervisor. I really think it varies on facility hiring. Some places you must have the certification. By doing the self study and learning on the job, I learned great skills from a handful of senior techs and I passed my exam on the first try. I have two friends that went through histo school and have each failed the test twice. My point? I feel it depends on the individual and where you are. In an interview situation, what you have are some tools and bits of information in a very contrived situation to give you some insight into how the person will act and perform as an actual employee. If your market or your organization, allows you the freedom to make your own hiring criteria, then good for you. But if there are legislative or organizational policies where you live or work that create stipulations, then that is how it goes. This discussion has rarely gone anywhere in the posts before, and seems to make more people upset than anything else. I guess no one appreciates being placed into some category. Hey, if you want to become certified, go for it, if you don't that is fine too. The question was about whether this affecting hiring- and I do think it does, in some places very much, in others maybe less? There are always a multitude of factors. Re: [Histonet] Cautery-Like Artefact
How long is your overnight processing run? Are these specimens being processed on an eight hour run, a four hour run, or something like a rapid run? On Monday, August 19, 2013, Roger Heyna wrote: We are experiencing a staining artefact on our HE's, most commonly seen on skin specimens, but also observed on other specimen types as well. Within the dermis and subcutaneous regions of the skin specimens, there are portions of what should be collagen that appear homogenized and stained with hematoxylin, instead of the usual eosin staining. It's often on the edge of the section and varies in size between different specimens. The pathologist is normally able to read around it, but there have been cases that were made more difficult to diagnosis by this artefact. Our pathologists believe this artefact resembles cautery artefact, but the dermatologists insist they are not using cautery during collection. We have a large derm service, and most of our derm specimens fix overnight. We have both microwave and conventional processing, and the artefact specimens have been processed on both. All of these specimens have grossing ink on them. Any thoughts on what could be causing this? If it's not cautery, does it seem like a fixation issue? Has anyone seen grossing ink affect processing?
