Does anybody know where we can purchase wire or plastic tissue cassette baskets 
for our grossing stations.  We are looking for baskets that will keep the 
cassettes in line similar to the old VIP 3000 metal baskets with dividers.  In 
our workflow we later transfer the cassettes to processing baskets.   (Looked 
into purchasing more processing baskets, but at 400-500 a basket that doesn't 
seem to feasible.)  Surely someone has old wire baskets or new plastic ones.

The test, according to CLIA, is the reading (interpretation/ diagnosis) of
the slide. You do not need a CLIA license to prepare the slides.

I agree with you, what happens when a lab processes only? We in the past 
have had accounts that we would gross, section, stain and coverslip. We 
then send completed slides to the submitting Physician usually a 
Dermatologist who likes to read and diagnose his own slides.

We were recently surveyed by the Joint Commission.  We have our
laboratory at one physical address and our Dermatopathologists at
another physical address (the hospital).  The surveyor indicated that we
need a separate CLIA and state license for the hospital location where
the slides are being read even though no tests are performed there.
These are high complexity Tissue Pathology.  Has anyone heard of this
before or have a similar set up and how are you licensed.  It doesn't
sound accurate to me.  My understanding was that any test being
performed required licensure, I have never heard of laboratory licensure
being required for the reporting of results.  Please provide any
assistance or experience you can!!! Thanks!

CLIA (feds) require blocks to be retained 2 years and slides to be retained 
10 years. Space usually determines how long beyond that they are kept. We 
keep a Cancer file of slides and blocks that are retained indefinitely.

I disagree, the slide is what was used to make the diagnosis. The block 
may not be a good representation of the actual slide, it may could have 
been cut so that what is on the original slide is no longer in the block. 
In Surgical path we keep our slides indefinitely and our blocks for 10 
years.

I read our SOP's on storage and referred to Dr. webb, our pathologist. His 
feeling was that they, AAVLD, leave that up to each lab depending on the 
state statutes regarding law suits. Our SOP says seven years on blocks and 
nine years on slides. I would agree with you that as long as you have 
blocks, the slides would be less important.

For veterinary diagnostic lab personnel:

We're having a difference of opinion here over how long our Histo slides 
need to be retained.  It was my understanding that all paraffin blocks and 
stained slides need to be retained for 7 years.  Others here believe that 
as long as the blocks are retained, it's not necessary to retain the 
stained slides past a year or two.  Storage space might be a problem in 
the near future.


I've looked through the AAVLD Essential Requirements, but cannot find a 
definitive answer to this question.  Can someone help me out with the 
correct answer?  We're writing a sample storage SOP for each lab section 
today, and I'd like to get an answer as soon as possible.
