It doesn't solve your problem, but I'm also using SSL for all IMAP
connections (with a cert. signed by an internal CA). Also, just to make
sure, I tried opening a few messages with large attachments (a couple were
2MB and another was 4MB). The largest one took several seconds to open the
first

Brendan, note the listen= argument in cyrus.conf, this is the name of
the socket that lmtpd will use.
In cyrus.conf: /var/tmp/imap/configdir/socket/imap
In error msg: /var/tmp/imap/configdir/socket/lmtp
Make them match and you should be good to go.
