I know of a 4th reason, that being that virtusertable is in use. There
may be some way around it, but everytime I use virtusertable it kills
subfolder delivery.

Whould it be possible to have an option in imapd.conf to let all users that
are created get this flag?
Maybe I'mwrong, but I seem to remember that this does not exixst today
(read through imapd.conf last night).

all subfolders of a user got this flag set. AFAIK the flag is set when the user
creaes a new subfolder.

AFAIK, the correct syntax would be:
deliver -m user.username.folder -u user
