we are setting up a Cryus IMAP server (2.1.18 on Debian Sarge). Account
management is done by saslauthd -a pam which gets its data from a
mysql database (pam_mysql). This works fine - with this setup the mail
users don't have to have a system account.

the name of the mail box that was specified on the deliver
command ?
What I have is procmail doing certain core tasks such as
spam and virus checking, expanding tnef attachments, etc,
and Sieve for end users to specify their filing
Got Cyrus+SASL2 working, but once I set the virtdomains flag I am unable
to create users with domains.

take another avenue in my attempt to get cyrus-imap working properly.

I too am struggling with this. Still haven't got it working as I like,
but have
got past the hurdle you describe.
You need to turn on unix heirarchy separator (sorry, forget the exact name of
this) and set 'virtdomains: userid'.
