I've tested this on kmenuedit from git master and cannot reproduce any of the
issues listed.

I've a similar problem and solved it this way:

1. look for the application ".desktop" file, like ~myhome/.local/share
   /applications/myprogram.desktop

2. edit line starting with "Icon=", e.g.
"Icon=/usr/share/pixmaps/myprogram.png"
   to "Icon=/usr/share/pixmaps/myprogram"

In short, I removed the extension ".png"

voila

I've been able to change icons on some items, but not submenus. Kubuntu 16.04
