My experience with this problem (KMail 1.9.9, KDE 3.5.9) is with IMAP accounts.
I have four accounts and identities (all gmail com) Whenever I reply to an
email in any of the accounts' inboxes, the correct identity is displayed in the
reply box, but the email is actually sent with the default identity.
This is a security issue!
