I really hate intermittent bugs. The previous two times (before just now) that I turned the AV receiver & TV back on, everything reverted to normal (VGA-0 off, HDMI-0 on at 1920x1080 60 hz at 0x0, DVI-D-0 offset to 1920x0). However, just now, turning the AV receiver & TV on resulted in: VGA-0

Would it be possible for you to test the latest upstream kernel? Refer to https://wiki.ubuntu.com/KernelMainlineBuilds . Please test the latest v4.18 kernel[0]. If this bug is fixed in the mainline kernel, please add the following tag 'kernel-fixed-upstream'. If the mainline kernel does not fix

Added 'xrandr' output after the AV Receiver and TV are turned off--note that VGA-0 has turned itself on, which shouldn't happen. ** Attachment added: "xrandr output after turning of HDMI accessories"
