I'm not sure how X orders the CRTCs, but theoretically that's just the
CRTC mask (i.e. which CRTCs can drive that connector). The order
they're printed in shouldn't matter...
If moving the actual CRTC made a difference, that would make more sense,
but that doesn't seem to be the case here.

The firmware is the missing part to support this device in kernel 3.16.
Unless we can get BCM43142A0-0a5c-216a.hcd from Broadcom, what we can do now is
just like what @Pilot6 said in the previous comment. If anyone wants to convert
an hex file to an hcd one, here's how:
I can confirm that this was fixed in kernel 3.19 (Ubuntu 15.04).
