Hello, on server http://www.hetzner.de/en/hosting/produkte_rootserver/ex40 with 
UEFI bios i have ubuntu with XEN  hypervisor and software RAID1. On kernel 
3.11-15 all works good, but on 3.11-17 i have this error on normal boot (normal 
option in grub):

normal and /proc/mdraid dont show any error.

So, kernel 3.11-17 is good, problem is with my kernel, what i attached.
I compile this and make package with this kernel becouse i need kernel
to virtual machine on xen pv. I installed this on host and in
/etc/default/grub i set default kernel 3.11-15 with Xen 4.1. After
install my package when i reboot server and dont touch keyboard, grub
automatical running kernel 3.11-15 with Xen 4.1 and print errors: md:
personality for level 1 is not loaded and errod with degradated raid
array from first post. When i manual select this same kernel, works
without problems. After remove my kernel from host all works good, why?

I have checked carefully, 3.11-17 is fine, so the problem is strictly
with the kernel attached above. I compiled and packaged it with the
intention to host Xen 4.1 server (pv). After installing said package
and rebooting, the system doesn't boot automatically.

What I mean is that during boot when the time alloted for user action
passes and no key is pressed, system boots with the default 3.11-15
kernel and returns

error: md: personality for level 1 is not loaded

while also complaining about degraded array, as I noted in the OP.

However, when I choose the very same default kernel with the
same options (default, not rescue), it boots up just fine. What can be
the issue here?

This may be a known regression.  Please try reverting these commits:

7dd09a1af2c7 xhci: replace xhci_write_64() with writeq()
e8b373326d8e xhci: replace xhci_read_64() with readq()

The reverts will be sent to the USB subsystem maintainer's tree today,
and will land in 3.14-rc2.
