This is a DKMS problem. It was compiling using the current running
kernel, i.e. the old one, rather than using the new kernel. I removed
all headers except for latest kernel and it still compiled against old
kernel. Eventually had to blacklist module for telephony card so I could
boot into new kernel without it hanging. Then did dpkg-reconfigure
dahdi-dkms which then compiled against current running kernel, the
correct one now. Then unblacklisted module and rebooted, all worked OK.
Not sure if this is a general DKMS problem or just the dahdi package.
Not sure how to categorise this bug now but I am working again now.

Bug description:
  If I boot on 3.2.0-43 kernel all works, any later kernels freeze. i
  have HP ProlIant ML110 G6 server running 12.04 LTS. I have 2 of these
  servers which I keep in sync. One has an OpenVox A400E telephony card
  in, this one crashes, the identical server except for this card works
  OK, although it also froze on kernel 44 but works OK on later
  versions.
