I have this bug too, on 16.04. After booting bluetooth works fine, then
after suspend it's gone, and I get the 'not enough handles' errors. Only
rebooting works. The solution of amias does not work for me.

Status in linux package in Ubuntu:
This bug affects me too, and I have the right firmware installed. After
resume, but not every time, the firmware fails to load. See for instance
this output from dmesg | grep -i blue:
