Have this problem on Ubuntu 16.04/xenial with discs on an eSATA-PCIe Card. It
uses an ASMediaChip ASM1062. BIOS Version is 0.95-something iirc.
All discs that are connected to this controller have this prob

The right thing to do here is to stop using read_threshold and find a
suitable replacement. Blacklisting a command to a drive because it does
the right thing when faced with an illegal command is ridiculous.

I was observing this problem. On Saucy an rsync backup would lead my
system to lock up. Something that did not happen on Quantal or Raring.
As a last resort, I removed the disk from its enclosure and, voila, I no
longer observe the problem.
