I gave 3.11.0-13-generic another try, and it worked fine. So I am going
to blame BIOS handling of iommu and a coincidence of power states.

Bug description:
  This morning, after first installing the package, I was uanble to boot
  linux-image-3.11.0-13-generic with default boot entries to have xen
  hvm 4.3 with iommu enabled. Without additional parameters, the boot
  process would hang (blank screen) where disks are normally mounted.
  After reaching the blank screen, it would not respond to Ctrl+Alt+Del.
  Hard reset was required. If I passed iommu=off or nomodeset as boot
  parameters, it would boot with xen. Booting without xen also worked
  normally.

  linux-image-3.11.0-12-generic continues to work with xen and iommu
  enabled without additional boot parameters. I have attached xen's
  dmesg under linux-image-3.11.0-12-generic  in case it is of interest.

Would it be possible for you to test the latest upstream stable kernel?
Refer to https://wiki.ubuntu.com/KernelMainlineBuilds . Please test the
latest v3.11 stable kernel[0].

If this bug is fixed in the mainline kernel, please add the following
tag 'kernel-fixed-upstream'.

If the mainline kernel does not fix this bug, please add the tag:
'kernel-bug-exists-upstream'.

If you are unable to test the mainline kernel, for example it will not boot, 
please add the tag: 'kernel-unable-to-test-upstream'.
Once testing of the upstream kernel is complete, please mark this bug as 
Confirmed.
