Just to emphasize something that Jon mentioned but is easily overlooked:
The icon must be authored in Inkscape. It cannot be created in Illustrator
and saved to an SVG file for use in Inkscape.
The reason for this is that SVG files can be created in many different ways
that give the same visual

Adding a new icon is certainly possible. There are some guidelines here:

In particular, the icon must be added with an appropriate SVG source file

Thanks, that was helpful: the 3DPivot.png is off the table. The request has
been modified to use a small sphere (a neutral shape), probably the same blue
color.
