ok, this seemed to be easier than I thought.
We are running Ubunto 12.04 so I did (on a test server first!):
apt-get update
apt-get upgrade
apt-get install koha-common (left the config files intact)
installed a few missing Perl modules like Test::WWW::Mechanize,
Archive::Extract
and now the

Thanks for that advice, it appears that the issue was with the fixed length
data field (specifically the 11th character for subject heading system)
being set incorrectly to an undefined value.
