If you do not apply the fix, you risk having duplicate records after mysql
restarts. Note that error already reported some duplicates.

Check the value of the branchcode for the item and the patron.

So the branchcode the the item is von Karman Institute Library and for the
patron is All branches.
The value of CircControl is the library the item is from.
