Which versions are you using?

I see:
  "Item level hold not allowed from OPAC"
and
  "Item level hold forced from OPAC"

Hi Katrin

Yes, staff can override, but they already get an warning when overriding
unallowed item level holds, whereas they don't get a warning when they override
unallowed bib level holds.

Hi Hugh, I think the item level/bib level hold settings only apply to the OPAC,
in staff it's always been possible to do both, assuming staff will have a
reason  asking for a specific item. But we could imagine a warning like you
suggest or maybe a note?
