due to upgrading host kernel from 2.6.37 to 2.6.37.1.
After that, some of guests started consuming 100% of CPU time and their os
seems to be stuck.
Looks like all the stuck guest were running 2.6.32 with kvm paravirt enabled.

Atomic-able hva_to_pfn() patches and allow-read-only-page patches
are merged almost the same time. But hva_to_pfn() does not handle
these two issues well together.
When @atomic !@write_fault host-is-read-only-page-mapped
the code will return fault_page, actually, it will be better
if we return
use EFER_SCE, EFER_LME and EFER_LMA instead of magic numbers.
