It could be a hook for making rag rugs. It has a fairly square shaped hook similar to the rug hook I have. Obviously hand make a perhaps made larger than normal for someone who had difficulty in holding the normally much slimmer handle of a shop-bought hook. Malvary in Ottawa -

U, right off hand I am tempted to call it an afficot (used to burnish threads in a finished piece of needlelace). The hook at the end is a bit of a puzzler but perhaps that is an artifact of a prior break in the bone. On the other hand, you could make a case for it being a primitive shoe
