In attempting to replicate a problem of a user on ask.libreoffice
(http://ask.libreoffice.org/en/question/52821/how-to-add-unique-index-to-a-table/)
I encountered a bad paint of the `unique` checkbox of the `index design` dialog
box in the Base table editing function.

STEPS TO REPLICATE:
1.  create a one-column table
2.  without creating a primary key go to index design
3.  create a new index
4.  click the `unique` check box in the dialog box

UNEXPECTED RESULT:

the checkbox has a check for a moment and then is unchecked and a second box is
painted to the right, both checkboxes having no check.

EXPECTED RESULT:

One checkbox with a check.

This bug seems to have a specific sequence of events to trigger, it does not
always occur just by clicking the box.

See screenshot.  Appears related to bug 92053 and bug 92110 but different
enough to merit a separate report.  Thus, seems related to use of KDE. 
Encountered in LO 5.0.0.2 using OpenSuse 13.3 /KDE4
