Oracle VirtualBox 4.3.10 + Ubuntu 14.10 server + LVM 2.02.98
Kernel:
4.1.0-rc7 (12 June, 2015 / Today) with your 4 patches.
Btrfs-progs: 4.0
Generic Test Procedure:
mkfs.btrfs -f -m $RAID -d $RAID && mount
