Have you ever tried with 'mkfs.btrfs -m single /dev/'?
As you had a RAID1 based on LVM, you don't have to keep
the default duplicated metadata
I was running this while the fs was mounted, which is probably not what
was expected, but here's a crash report anyway. :)
