Don't handle MJPEG pixel format as JPEG. (thanks to Hans de Goede) Authorize reqbufs ioctl when streaming. Bad mutex unlock in poll() when streaming problem. (thanks to Mauro Carvalho Chehab) Don't handle 'webcam disconnected' in stream off.
