Does anyone know how I can use ERT to produce a 3-em dash like that
used in bibliographies by the _Chicago Manual of Style_?

You can just use --- without any ERT. TeX turns it into a longish
dash, which may be what you want...

Two choices: 1) Edit-Math-Math Panel and select the symbol from there.
Or, 2) look in the comprehensive LaTeX symbol list, available from CTAN. I

displayed in lyx (1.3.4) inside a grey box along with et al., and the
year. but for some reason there is not whitespace between the author
name and the et al.

*.lst style, but i cannot find the way to do it inside lyx (1.3.4).
instead, i must export to latex and execute the makeindex command in a
console. is there any way of doing that inside lyx?
