I don't use Mac so I cannot be of much help here. I would suggest that
you read Help > Customization > Section 5. Otherwise, try this:

I do not have actuarial class installed, so I cannot test it. But you
may try the attached LyX file. Normally you should be able to insert
pure LaTeX or TeX code in ERT boxes (Insert > TeX Code).

Hello, I'm a newbie in LyX and LaTeX, I'm writing a thesis in actuarial
science.
I'm using LyX 1.6.4.1 on Mac OS X Version 10.5.6.

Please take a look at page 108.

I exported my LyX document in *.tex and then I pasted the code from the pdf
in the tex file. Then I saved the tex file and I import it back. Then LyX
always crashed down. 

Can somebody tell me what to do? I am really thankful if someone can put the
code in the right area and send me a LyX file... that would be very, very
awesome.
