You need a layout file or a module. If you are lucky, there is a module
providing this - either in the list of modules at
Document>Settings>Modules or somewhere at wiki.lyx.org.

Otherwise, you need to read the Help>Customization guide about writing
your own.

How do I add an item to LyX format menu? Usually I use "Theorem" or "Lemma"
but this time I need "Question". It doesn't appear in the drop-down menu and
I don't know how to add it.
