As soon as you put it in a float, that’s when I get the effect you
describe, so try Inserting the Table on a blank paragraph in normal text
mode.
The table has 84 rows and needs to be handled by longtable. Right clicking
on the header row and selecting Settings -> Multipage tab I set multipage
'on' and set the selected (header) row to be the header row on all pages.
Looking at both the dvips preview and the pdflatex .pdf file the table

Off the cuff, I'd put it on a right flushed multicolumn and mark it as
footer (or whatever it's called in the menu). Then you need a line a 'last
footer'.
