TOC update

QT interface, long book Ms, single file, undergoing final editing for a
publisher. This file is also under RCS control with many saved versions.

An open and expanded TOC (for easy navigation of large documents during
editing), closes up under many conditions. I have identified two, one of
which is just dumb and annoying.

Conditon 1: when editing a heading that is in the TOC. This ALMOST makes
sense, it is at least comprehensible.

Condition 2: when I move (cut and paste) material around within a
paragraph, an expanded TOC collapses upon paste. VERY ANNOYING and
not very comprehensible.

I have not searched for other conditions, but have found the TOC collapsed
when doing different types of editing.
