You just have to *uncheck* the box "Use AMS math package automatically" and
then check "Use AMS math package" in Document -> Settings... -> Math
Options. Then Apply. Worked for me in LyX 2.0.3 with "article (SPIE
Proceedings)" Document Class.

Why haven't you followed my advice to set the AMS option as I wrote? Doing this, subequations will 
be known by LaTeX as I explained.


Besides this, there were was a mistake: You set the paragraph of the subequation formula to 
left-aligned. Doing this is not allowed when using subequations. (I'll add this info to the Math 
manual.)

Two hints:
You don't need to use TeX code for vertical spaces; LyX supports them via the menu 
Insert->Formatting->vertical space.
You also don't need to use TeX code to insert characters like the copyright sign ©. You can insert 
them via the menu Insert->Special characters->Symbols.

But I have explained you why this is the case in my email.

You need to assure that you have set in the LyX document settings the 
math option "Use AMS math package".
(I guess that in your other research papers you used the option "Use AMS 
math package". This worked because you had other AMS constructs in your 
file that LyX directly supports. In this case the AMS LaTeX-packages 
were automatically loaded. subequations are not yet directly supported, 
therefore the AMS-package are not automatically loaded.)
