Ok. I stand corrected. what I did was disable vo and hit space on the ok button
and reenable vo after about 45 seconds. the user was created.

It's not vo. It's a bug in the system prefs. This has been around since I
believe the new update to 10.9.0. I reported tho to apple through their bug
tracker and they told me to submit a diag thignwhich I did.

I have made this bug known to Apple's software engineers.
