Are you speaking the commands to calibrate while your Escape key is pressed?
The Escape key normally needs to be down in order to both calibrate and to
issue commands unless you’ve set it differently in the System Preferences.
There shouldn’t be anything else you need to set anywhere
Try turning Speakable Items off and back on again, or switch the listening
method from pressing a key to always on. I made a podcast about this and
dictation for www.applevis.com, but I don't know if it would be helpful or not.
Also, are you running Mavericks? In older versions of OSX I found

I think this window is meant to show you what method the mac is using for
listening and maybe even visually that it hears you speak.
It is the speech feedback window if I am not mistaken and I think there is a
command to say minimize speach feedback window.

You won't get a sound when you do this, you will only get one if the command
you give is understood. Also, make sure the input source is set to the

to start a command avoids hanging the system as does using the constant
listening feature.
