The problem you described has been bugging me for months, on my 8 Plus, also.
After reading Pete's subsequent reply on this, I discovered that in order to
get speaker phone activation to work, I had to first disable the Select Speaker
setting, in VoiceOver options, then re-enable

I use Aftershokz for my preferred bluetooth headset when out and about. If
I’ve not used the headset for a while (let’s say a few weeks) but they are
paired to my I phone 12, I find that the calls that I receive are not going to
the headset. I have been in the audio settings and can’t
No utility is required because a terminal command easily does it; which the
utility likely duplicates. While in terminal and in the flash
drive do this command which removes all .folders or single .files, make sure
none of them are neededd as are some in a users home directory.
