Hi. I hope this helps. I have both El Capitan and High Sierra
computers. El Capitan seems to still work as expected. When I save attachments
I go to File then down to save attachment. Then a dialog box pops up allowing
me to choose where to save.
In High Sierra what appears to happen is

On occasion, I've had the Mail app not bring up the Save dialog right away. In
some cases, I believe that it was just a VO focus thing, and in other cases, I
just had to repeat the request. That is, if it was a focus thing, cmd-tab out
of Mail then cmd-tab back in and the dialog should be

this was also my thinking, but I checked the downloads folder. and the
attachment was not there. I did a spotlight search for the attachment, and the
only hit I got was the email message with the attachment, not the actual
attachment itself.

Really? I'm guessing they're going to your downloads folder, then. I thought
there was a way to change that to prompt for a location, but I'm not in front
of my Mac just now to verify.

Thank you for your response. However, accessing the save attachments from the
menu does not bring up a dialogue box asking me where to save it. When I
select that option, the menu disappears and I am brought back to my message in
focus.

It sounds like VO isn't focused on the attachment. Also, remember that
attachments may not appear at the end of messages. Some mail clients, including
the default ones on macOS and iOS, will embed attachments wherever the author
wants them.
