Not entirely clear on which system you are testing with. On the Macintosh, if one is in web group navigation instead of Dom navigation (as set in the VoiceOver Utility) then often times VoiceOver reads things in the incorrect order. For example when asking VoiceOver to read a paragraph with a link in it, the link might get read after the paragraph rather than inline.
I haven’t really paid much attention to how text added by CSS is being handled.
