In general, if accessibility isn’t mentioned on the Web site of an application, then it isn’t accessible to screen reader users. I’m sure there are exceptions, but, in my experience, this is usually how it goes. Accessibility doesn’t occur by accident. The forms that I’ve encountered let you set check boxes and activate a button to provide an electronic signature (e.g., DocuSign). Find out whether the system you’re using has an accessible option. I think I got it to work once with the Track Pad by writing my signature in the box.
