When you add an attachment to a journal entry but don't yet have a
folder to place the file in, you need to go to the files area to create
the folder and then upload the file and go back to the journal or you
upload the file and then go to the files area to create the folder and
move the file into it.

This is very cumbersome. It would be better to be able to create a folder 
directly in the journal entry's attachment area. This should also be possible 
in the other instances where that code is being used:
- Files to download block
- Image block
- HTML block
- Embedded media block
