We had a CAPTCHA until 1.3 and we took it out because CAPTCHAs don't
really work all that well anymore (spammers hire humans to defeat them)
and it broke our accessibility.

Ah. So that is deliberate. OK. Makes sense. However, then it doesn't
work very well because so far I have only seen the two variations that I
listed above: Only email and first name get switched.

I don't think it has anything to do with institutions. Since the anti-
spam stuff came in, the order is randomised every page load to make
things a bit harder for bots. Contact us is similar.

This is actually a feature of the new anti-spam protections that were
added in 1.3.
If you reload the page, it will randomly swap the order of these fields
to prevent automated scripts from using the registration page.
